4. Copy of most recent bills (credit report AND bills are necessary for getting debt amount and address for sending notice of bankruptcy. If debts have gone to debt collection, have debt collector and their attorney contact information provided also).

5. Copy of 2 most recent pay stubs

6. Credit Counseling Certificate (CCC) (we will discuss this when you call)
